Financing Your Care
In our office, we offer high quality care and payment options that work for all of our patients. Below you will find a list of insurance and payment plans that we accept. If you don’t see your preferred method on our list, please call our office at Appleton Dental Phone Number 970-242-9207 to find out what other options may be available to you.
Dental insurance plans we work with:
- We will submit your claim to any insurance company whether we are in-network or out-of-network providers for that particular company
- We are contracted with Delta Dental of Colorado as a Premier Provider
Payment options:
- Paid in Full Courtesy: If we are not billing insurance for your visit, a discount of 5% will be subtracted from your total obligation if the entire treatment balance is paid in full, with cash or check on the date of service.
- Credit Cards: We accept Visa, Mastercard, American Express and Discover.
- Payment plan: Please contact our office to discuss.
Other options:
- Care Credit: For those who prefer an extended payment plan. Ask about our 6-month, no-interest contract with Care Credit. You may apply quickly and easily either by phone directly with Care Credit at (800) 365-8295 or online at www.carecredit.com.
Please note: Payment is due at the time of service.
As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
